Identification:

Identification requirements have been intensified since September 11, 2001. Three pieces of identification are required for each applicant. A social security card and either a certified birth certificate or out-of-state license must be presented along with two of the following additional documents:
  • Passport
  • Military ID Card
  • State-Issued Non-Driver ID Card
  • Resident Alien Card
  • Certified Marriage License
  • Original US Military DD-214
A school enrollment form is acceptable along with a social security card and birth certificate for applicants under 19 years of age.

Requirements:

Learner's License:

  • Certified document proving name and date of birth (Bureau of Vital Statistics - Birth Certificate).
  • Pass a written exam ($5.00 Cash Fee Only) and eye exam.
  • Learner's License cost ($23.00 check or cash).
  • New Driver's License: Complete requirements for learner's license and pass road test.

Out-of-State License Applicant:

  • Picture license or clearance letter from former state.
  • If valid license ($5.00 Cash Fee Only) and pass eye exam.
  • Cost of license ($23.00 cash or check).
  • If out-of-state license is expired over three years, pass written eye and road tests.

Duplicate License:

  • All Duplicates $15.

Renewals:

Driver's License Renewals are available at the Madison County License Department

Vessel License:

  • Age exempt if age 40 on or before April 28, 1954, exempt from test;
  • All others must test or provide a boating course certificate.
  • All must pay $5 cash fee. Must be at least 12 years old to apply.

  • Class D/Operator $23 Four years
  • Class M/Motorcycle License $23 Four years
  • Class V/Vessel (only) $23 Four years
  • Motor Driven Cycle License $23 Four years
  • Learner's License $23 Four years
  • Class A Commercial License $48 Four years
  • Class B Commercial License $38 Four years
  • Class C Commercial License $18 Four years
